Chronic Disease & Diabetes

The Woods Medical Centre can offer individualised care plans for those with chronic disease such as Diabetes (Chronic Disease Care Plans, also known as GP Management Plans). 

Chronic disease, as defined by the Australian Institute of Health and Welfare (AIHW), are ‘long lasting conditions with persistent effects.’ These types of diseases are becoming increasingly common in our country, with more than 11 million people suffering from at least 1 of 8 chronic diseases in 2014-15.  

The main types of chronic disease include:  

  • Musculoskeletal conditions such as arthritis and osteoporosis

  • Respiratory diseases including asthma

  • Back pain

  • Cancers such as lung and colorectal cancer

  • Cardiovascular disease

  • Chronic obstructive pulmonary disease

  • Diabetes 

  • Mental health conditions including depression  

The Woods Medical Centre offers Chronic Disease Management services to patients living with these conditions. 


Diabetes is a serious and complex condition which can affect the entire body. Diabetes requires daily self care and if complications develop, diabetes can have a significant impact on quality of life and can reduce life expectancy. While there is currently no cure for diabetes, you can live an enjoyable life by learning about the condition and effectively managing it.

There are different types of diabetes; all types are complex and serious. The three main types of diabetes are type 1type 2 and gestational diabetes.

Our doctors can prepare a specific review of your health needs and your care plan can help you access medicare benefits for allied health visits (e.g. Dietitian and Exercise Physiologist consultations) that may help in monitoring your diabetes (including cholesterol,  blood pressure, kidney health, glucose and body weight) and help prevent complications.
